Responsibilities & Duties:
Providing intensive care management to foster care youths of all ages
Communication and relationship building with youths, foster parents, and collateral partners
Development if interventions and goal setting
Participate in monthly team meetings
Document encounters daily in conjunction with quarterly paperwork and care meetings
Provide after-hours on-call support on a regular, rotating basis
Required Skills & Qualifications:
Bachelor's Degree in the human service or behavioral science field
LSW, LSW-A, or license-eligible
Familiarity with trauma-informed care
Valid driver's license and ability to clear DMV, DCF, and CORI background checks
