Join a collaborative, therapist-led team at Avamere Three Fountains, a 117-bed skilled nursing facility with a spacious therapy gym, kitchen, and access to a health club and swimming pool for therapy sessions. We offer in-house therapy services across Avamere communities in Oregon and Washington-no travel between buildings required.

Your Role:

• Carry out treatment plans under supervision of a Physical Therapist
  
  
• Document care per facility and insurance requirements
  
  
• Communicate with patients, families, and interdisciplinary teams
  
  
• Report changes in patient status
  
  
• Participate in department operations and development
  
  

What You Need:

• Graduate of an accredited PTA program
  
  
• Unencumbered license and in good standing with licensing boards
  
  
• Knowledge of Medicare and payer regulations
  
  
• CPR certification
  
  
• Professionalism, strong communication, and multitasking skills
