Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery. Knowledge of the OASIS process. Knowledge of Community Resources. Ability to make meaningful observations, evaluations and reports. Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships. Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment. Ability to read, write and speak the English language in an understandable manner. Ability to bend, push, pull, move and/or lift Must possess a Nursing degree from an accredited College or University and be a Graduate of a CT approved RN program.
